Yes, we have transportation available to your appointments outside of the facility.

Yes, so long as the items fit in the room and don’t cause any difficulties with mobility or providing care.
Yes! We love it when well behaved pets come to visit! Please be sure to bring your pet’s vaccination records when they come to visit.

Decided that your loved one is no longer safe living independently is a difficult thing to do. Give us a call, and we can help walk you through the process. Also, visit the Resources section of our website to find some links that may be helpful to you.
Bay Bluffs is a 24/7 facility, and does not have set visiting hours. If you’ll be joining us for a meal, please see the receptionist to purchase a meal ticket.

Please bring:

  • DPOA, Guardianship or other legal paperwork
  • Medicare, Medicaid, Insurance and Prescription cards
  • Admission paperwork- it MUST be signed prior to or on the day of admission. The responsible party must be in attendance, or have made prior arrangements with the Admission Coordinator
  • Four sets of clothes, three sets of sleepwear, six sets of undergarments, seasonally appropriate outerwear, comfortable clothes for exercise, including footwear, and any personal toiletry items that you or your loved one prefer
